Act now, the following changes must be reported to OPG

Either the donor or one of the attorneys must inform the Office of the Public Guardian in the event that

1. You or an attorney change name or address
2. An attorney dies

If there is a name change you must tell OPG. A copy of the marriage license or deed poll showing the new name must be sent.

If you or your attorney change address you must tell OPG. In this case you do not need to send any documents to show this.

If one of your attorneys dies you must tell OPG and send them the original LPA and any and all certified copies. If your attorneys are able to make decisions ‘jointly and severally’ then OPG will update the LPA and return it. Otherwise, the LPA will be cancelled as the named attorneys will no longer to be able to make a joint decision.

When contacting the OPG you should include:
– Whether you are the attorney or the donor
– Your full name, address and date of birth
– The donor’s full name address and date of birth (if you are the attorney)
– The reference number on your LPA
