Form LP3
This is used to update anyone specified as a person to notify in Section 6. See the form on the government website here
Form LPA120
This is used to apply for a reduction or exemption for the LPA fees from the OPG. You will also need to provide evidence of your circumstances. Form LPA120 can be found on the government website. Click here
Mental Capacity Act
The LPA empowers attorneys to make decisions for the donor in the event that they cannot make decisions for themselves. The LPA is governed by the Mental Capacity Act (2005) and attorneys should follow the guidance in the official Code of Practice.
